Can someone shed some insight as to what happens to the data and folder structure when a module is deleted?

Sure, the folder the module is in gets deleted, along with it's assemblies, if the module provided a sql script for uninstallation, this is also run (this may clean up any date and tables, but is dependant on the module implementing it)

Scott, do your Modules include a SQL Script for the database clean-up? How does one find out if a module contains a SQL Script when it is installed on the portal?

It should be in uninstall.sqldataprovider.

It will only clean up if the actual module is deleted from host -> module definitions.
