I know this sounds strange, but I have been looking for a while, for a Contact Module that i can customize. I am going to use the form, for people to submit Links to our site.

Basically, I will set up the following Questions:

Link Address: (Short Textbox)
Site URL: (Text box)
Site Description: (multi line text box)
Link Back URL: (Text Box)
Your email address: (Text Box)

Then a button that says "submit your link request". After the submission, text comes up "thanks for your submission". An email with the above information will be sent to the ADMIN of the site.

Easy Yes, but I have only found complicated modules to set this easy thing up?

I have DNN: 3.3.7

I like XMod to create any contact or feedback forms.
It gives you full flexibility to setup almost any form you can think of.
You have to decide yourself if it's easy enough for your example though...

XMOD. Once you have created one form, it is ia simple matter of customizing the code to create dozens of others. Kelly, at dnndev.com, is a great developer and the product is every bit as good as Scott's products. We all know how good they are!

With xmod forms, you have the option to send the results to various email accounts, re-direct to a url including passing form parameters, export to excel (version 5.0 coming soon), and use constants including most of the DNN parameters (userID, userEmail, portalID, etc.)

Thanks for the feedback. I am actually an owner of XMod as I have heard it can do some great stuff. I guess I was a little overwhelmed once I saw the 390 page manual.

Time to get my hands dirty and figure out what everyone already knows :)

I really like Form Master.

Easy to setup, reports are run within the module interface and exportable.

It would literally take 5 minutes to setup the form you described.

I also own XMod and found it far to complex for simple forms.
